HIGH SCHOOL CAMP

JULY 23-28, 2017

 

Summer Camp 2017 will be held at the beautiful Triangle Lake Camp (formally known as Eagle Cove Camp) nestled in the foothills of the Siuslaw Valley about an hour east of Florence, Oregon.

STEP 1: Register your Student

  1. Begin by saving the Camp Registration form to your computer.  This is a fillable pdf form that you should keep for your records.  Right click on the link above and select “Save link as…”. Then select a safe place on your computer to save the file.
  2. Go to the place you saved the file and double click on it to open the fillable pdf form in Acrobat Reader.  If you do not have Acrobat Reader you can download it for free here – do not install the optional offers.
  3. Fill out page one of the form and sign it by typing in your name and date at the bottom.
  4. If you need to add anything specific about your student, use the second page to add additional information regarding medications, allergies and other medical conditions we need to know about.
  5. Save the form by clicking on the Save File icon that looks like a disk in the upper left icon bar.
  6. At the bottom of page 2, there is a link to email the form that says Submit. Click on it to email the form you just completed.
  7. If for some reason the email link does not work, manually email the pdf form as an attachment to JoanneF@ocec.net and use the Subject Line –  High School Camp Reg Form.

STEP 2: Pay for Camp

  1. Early Bird Registration runs through 6/25/17 and a break in price is offered. Not Available at this time.
  2. Standard Registrations run from 6/26/17 through 7/16/17, standard rate applies. Not Available at this time.
  3. Late Registration runs from 7/17/17 through 7/23/17, extra fees apply. Not Available at this time.

STEP 3: Fill out Medical Form

  1. Make sure your medical form is up to date. This is a fillable online form that is sent securely and confidentially only to OCEC and is good for all youth activities.
  2. Initial the first part of the form by clicking on the button that says Initial, a window will pop op for you to use your mouse and draw your initials.
  3. Fill out the next pages and once you are done, click on the Agree to the this Document button at the bottom of the form.
